Request the current distribution packet from the current provider
Ask the plan administrator, custodian, or recordkeeper for the current distribution packet for your account. The packet should identify the available payment types, the plan notice, the election form, and any special process for beneficiaries or alternate payees.
Use the current provider’s documents. A form from another employer, a prior recordkeeper, or an old online search result can describe a different plan or an expired process.
Confirm you are making the right type of request
Before filling in an amount, identify whether you are requesting cash, a partial cash distribution, installments, a direct rollover, or another available payment form. A direct rollover sends eligible money to a receiving retirement account; a cash payment is made to you. The instructions, payee details, and withholding treatment are different.
For a former employer plan, ask whether the entire balance is available and whether a partial payment can be requested. For a current employer plan, ask which in-service transaction the plan allows. The answer should come from the provider’s current materials, not the balance shown online.
Complete the election from the provider’s instructions
The form may ask for the account source, gross amount, federal or state withholding election, payment destination, signature, and consent information. Do not guess about a field that changes the payment type. Ask the provider what it needs before submitting the form.
If the request includes a direct rollover, obtain the receiving institution’s exact name, account format, and delivery instructions first. If you are receiving cash, ask what amount will be withheld, how the payment will be sent, and which tax form will report it.
Keep a transaction file while the request is in process
Save the statement, distribution notice, completed election, identification or consent confirmation if applicable, provider messages, confirmation number, payment record, and later Form 1099-R. Those records establish what was requested and how it was reported.
The provider can give its current processing estimate and explain whether any item is missing. It should not be necessary to submit duplicate requests while a complete request is being reviewed.
