What to do when a distribution request needs more information or is declined

An incomplete record or a declined request is a reason to identify the provider’s stated issue, the account facts, and the next required document, rather than guess at a new request.

Get the provider’s reason in writing

A distribution request can stop because the account is not identified correctly, the requested payment is not available under the plan or contract, a form is incomplete, a signature or consent item is missing, the receiving instructions do not match, or the provider needs a document connected with ownership. Ask for the current written reason and the exact form or instruction that applies.

Do not solve an unclear response by filing a different payment request at random. First determine whether the provider is asking for a correction, a different transaction type, additional documentation, or confirmation that the payment is available now.

Rebuild the account record from the documents that exist

An old account can often be reviewed even when records are incomplete. Start with any plan name, employer name, account number fragment, recent or older statement, prior Form 1099-R, rollover confirmation, and correspondence. The point is to give the provider enough accurate facts to locate the account and explain the available process.

If an account changed providers or the former employer no longer operates, identify the current recordkeeper or plan administrator before completing a form intended for the old institution. A current provider contact can explain its records and request requirements.

Recognize when the question has moved beyond a provider form

A provider can explain the plan’s forms and distribution choices. It cannot resolve every tax filing, legal authority, divorce-order, estate, or investment-selection question for the requester. When the obstacle depends on one of those issues, the relevant professional work should be completed before the account request is resubmitted.
